Qualifications for
Purchasing Materials
The
Placing an order
Please submit a completed Order Form.
Orders are accepted by email, fax, mail, or phone. If this is your first order, you will also
need to submit a completed Customer Qualification Form. Once we receive your completed Order Form, we
will calculate total cost including shipping and handling and any applicable
taxes. We will then send you an email
with order confirmation and payment instructions for your final approval. After receiving your approval and payment,
your order will be promptly shipped.

Purchase Orders from U.S. government
agencies or academic institutions may be accepted. Shipping/Handling charges (see below) must be
included. Pre-approval is necessary for
the use of a Purchase Order. Please
contact us for pre-approval.
Prices
Prices are
subject to change without notice. All
prices are in U.S. dollars. Texas
residents must also pay 8.25% sales tax.
Shipping and Handling
Shipping
and handling charges will be added to the listed price of the items at the
following rates:
Regular
shipping rates:
U.S.(Continental):
10% of the item’s cost, minimum $9;
Non-continential
U.S.: 10% of the item’s cost, minimum $20;
Canada and
Mexico: 10% of the item’s cost plus country specific shipping charges, minimum
$20;
Others: 15%
of the item’s cost plus country specific shipping charges, minimum $20.
Regular
shipping (Continental U.S.) includes:
U.S. Postal
Service and Fed Ex ground service
Expedited
or express delivery:
Available for
additional shipping charges. Please
indicate on your order if you wish shipping to be other than our regular
shipping method.
Foreign
delivery (non-U.S.):
Please
contact us for country specific charges.
Payment
All orders
must be prepaid by check or money order in U.S. funds (made to “The
Neuropsychology Center”) or by credit card (Visa, MasterCard or Discover). Wire or ACH transfers will incur a charge of $15
(domestic) or $25 (international).
Returns
Returns of
unused and unopened items are allowed up to 30 days following purchase. You must obtain a Return Merchandise Authorization (RMA) number
within 30 days following purchase. The Neuropsychology Center will not accept
returns without prior authorization and an RMA number. Once issued, RMA numbers are valid for 15
days. The Neuropsychology Center must
receive the returned products within this timeframe. RMA numbers will not be extended or
reissued. Customer should include the
RMA number on a letter indicating reason for return. A full
refund (minus shipping/handling charges and 15% re-stocking fee) will be issued
once RMA items are inspected for condition.
The
Use of Materials
Purchaser
agrees to use the materials in their intended manner. Purchaser is responsible for their use, and
The Neuropsychology Center, P.C. is not liable for any problems which arise in
the use of the materials. Purchaser
agrees not to copy materials and not to release them to any unauthorized user. Purchaser agrees to maintain test security
and thus agrees not to disclose information about the materials that would
serve to decrease their usefulness (as novel test stimuli). User agrees not to include unnecessarily
specific information or details about the test materials or procedures in
written test reports.
Acceptance of Terms and

By submission
of an Order Form and Customer Qualification, the purchaser agrees to these
Terms, Conditions, and Qualification Policies.
